To register with the Practice you need to complete a registration form & a new Patient Health Questionnaire. Please call into the surgery for one of these forms.
If you are registering as a new patient with our Practice you will need to provide us with photographic proof of identification in the form of a current passport or driving licence. Anyone registering with the Practice from abroad will need to provide proof of residency i.e. work visa. We reserve the right to deny registration
After registration you need to contact reception to make an appointment to see the Practice Nurse for your New Patient Health Check
You will need to complete one of these forms for each person you wish to register with our Practice.
